How long have you been employed in the Leather and Hide industry?
(A widow(er) should give details of his/her spouse or civil partner’s employment in the industry).
Please give as much information as possible.

Please include all regular monthly/direct debit expenses, such as rent, mortgage payments, council tax, loan repayments, utility bills, insurance, telephone and internet. Do NOT include food, clothing or other day-to-day costs.

• After adding an expense category, such as council tax, and completing the relevant monthly cost, click the button to add a new row to detail the next expenditure category, for example, utility bills.  Continue to add rows until you have completed all the relevant expenditure categories.
• If you need to delete an expenditure category, click the button to remove the incorrect category details.

Please include all regular income receipts received into your bank account, such as state and/or private pension, pension credit, any other state allowances or benefits including universal credit (list all items individually), investment income, employment (net of tax), any other regular income received.

• Click the button to add additional Income/Savings/Liability items. Click the button to remove an item added in error.

The posssession of property or other assets will not necessarily prevent assistance being given but it is essential that full knowledge of such assets should be made available to enable the Committee fully to appreciate your circumstances.

Please include all savings and investments owned by yourself or your spouse or civil partner. For example, Building Society or Bank savings accounts, cash or equity ISAs (estimated value), premium bonds, shares (estimated value).

Please include all loans and other debts (excluding mortgages disclosed in section) owed by yourself or your spouse or civil partner. For example, bank or friends/family loans, credit card balances.
